Summary0000554: interFoam outlet boundary reflects phase if situated in positive coordinate quadrant
DescriptionAll different kinds of outlet boundary conditions result in a reflection of the first phase at the outlet if the outlet is situated at a place with positive coordinates. Does not appear if situated at vertices with negative coordinates.

The two pictures show the simulation at the same timestep and only differ in grid location, the one without phase reflection has its grid located with negative x-coordinates!

I guess you are using a fixed p_rgh condition at the outlet, you may find that using zeroGradient is more physical. If this is not satisfactory you will need to develop an approapriate transmissive condition for you outlet with careful handling of the p_rgh field.
